Providing Quality Care Outside the Clinic

We know that not everyone who needs care has the means to come to our clinic. Through the Health-Bridge program, FCHC collaborates with community partners to bring essential care to our neighbors in need. FCHC medical providers travel to shelters and ministry offices, make house calls and utilize a mobile clinic to provide medical services in safe and accessible locations to our most vulnerable patients

Through Health-Bridge, FCHC serves people experiencing homelessness, mothers and their children in crisis, patients overcoming addiction and survivors of domestic violence.

Bringing Providers Where They’re Needed Most

Through Health-Bridge, our physicians are able to treat patients in a space that’s familiar and comfortable to them. For people going through a difficult time, it’s important to have a physician they can trust and rely on a medical team who cares to go to them. All Health-Bridge services are free of charge to our patients.

Beginning in the summer of 2025, FCHC will have access to the Rappahannock Area Health Department’s new Mobile Health Clinic. Through the use of the mobile health clinic, FCHC will be able to partner with additional organizations and bring care to the rural areas of our community.
